Studies including the University of Manchester and their employees also require a Pan-Man Notification form to be completed. Guidance notes for the Pan-Man are also included here. This form needs to be signed by various parties and the original fully signed form needs to be sent to the R&D Department. Please keep a copy of the form for your own records.
